New Patient Information

Most likely, you have been referred to Eastern Pulmonary, Sleep & Allergy, P.C by your regular physician (self referrals are allowed if your insurance approves). Your primary physician provides for your other health care needs, allowing us to concentrate on our specialties.

The goal of this website is to help you learn more about how our services can help you. It will also answer many of the questions that new patients frequently ask.

What to Expect On Your First Visit

Your first visit will generally consist of obtaining your detailed medical history, performing a physical examination, and gathering additional information through laboratory testing. If we need to perform more sophisticated tests to correctly diagnose your condition, we can perform them within our office and without hospitalization.

In order to save time and reduce your medical costs, please bring the following information with you:

  • Previous medical records and reports
  • The actual chest x-ray film
  • CT and CTA scan/ECHO/ultrasound CD (if available)
  • All of your medications or a detailed list

By providing these materials, not only can you save time, but it also helps us to avoid duplicating tests that have already been performed for you.

In addition to the above items, please bring all of your insurance information (insurance card) and photo I.D. so that we can assist you in filing insurance claims.
